A. They always err and are led astray in their hearts
"And so I was provoked (displeased and sorely grieved) with that generation, and said, They always err and are led astray in their hearts,"
Err means: (Greek word: "planao") [verb]
1. to cause to stray, to lead astray, lead aside from the right way
2. to lead away from the truth, to lead into error, to deceive
3. to be led away into error and sin
Hearts means: (Greek word: "kardia") [noun:feminine]
1. of the soul so far as it is affected and stirred in a bad way or good, or of the soul as the seat of the sensibilities, affections, emotions, desires, appetites, passions
2. of the will and character
3. of the understanding, the faculty and seat of the intelligence
4. the soul or mind, as it is the fountain and seat of the thoughts, passions, desires, appetites, affections, purposes, endeavours

Even Israel is GOD's Chosen People and Nation, they don't believe in GOD, they don't come to Him by faith. They are like unbelievers of the Word, even they heard the Truth, they rejected knowledge of the Truth.
B. They have no relationship with the LORD

95:10 Forty years long was I grieved and disgusted with that generation, and I said, It is a people that do err in their hearts, and they do not approve, acknowledge, or regard My ways.
Psalms 147:20 (Amplified Bible)

They simply have no relationship with the LORD. Why have relationship with Someone you don't know? The bottomline is know GOD first thru His Words, grow in the knowledge of GOD and then have a deeper relationship with Him.
IX. CONCLUSION:
We are children of GOD, if we have known GOD, continues to grow in the relationship-knowledge of GOD and have relationship with Him as a FATHER and we are sons.
